The Crescent Community Clinic is a recipient of the 2018 Community Outreach Grant Program.  On October 18, 2017, the Clinic held their annual "Choose Health" event.  The requested reimbursement from the Clinic is $1,205.87.  Because the Clinic did not use the entire $5,000 grant award, they are requesting to use remaining funds for an additional event for the homeless on December 21, 2017.  The costs for the additional event is estimated at $1,700.  Because the second event was not included in the original grant application, staff is requesting the Board approve.  If approved, this additional request will be subject to the Community Outreach Grant policy.  Attached for your review is the request from Crescent Community Clinic.
